WINTER SPORTS
Paperwork Due: Oct 22th, 2021

Boys Basketball

Tryouts: November 1 - 3

Time: 3:15pm - 5:45pm

Location:
11/12 Grade - New Gym
9/10 Grade - Old Gym

Arrive at the facility by 2:45 and ready to go. Short meeting before tryouts at 3:00pm.

Girls Basketball

Tryouts: November 1 - 3

Time: 6:30pm

Location: New Gym

Bring your own water

Boys Soccer

Tryouts: November 1 - 3

Time: 3:00pm - 5:00pm

Location: Cypress Park

Bring cleats, water, shin guards, and running shoes

Girls Soccer

Tryouts: November 1 - 3

Time: 6:00pm - 8:00pm

Location: Lower grass practice field (HS)

Please bring a ball, cleats, water and a great attitude

SPRING SPORTS

Baseball, Softball, Tennis, and Track

Tryouts: starting Feb 7th, 2022

Paperwork Due: Jan 28th, 2022

 

Mandatory Paperwork

The following must be completed and on record in the Athletic Office PRIOR to a student being allowed to tryout/practice and/or participate in athletics.

1. A completed 21-22 AIA sports physical (5 pages, must be done after March 1, 2021)

2. Two emergency cards (parent and student signatures)

3. Handbook form, Informed Consent & Enrollment Verification (parent and student signature)   |  watch Informed Consent video  |

4. AIA Concussion form (parent and student signature)

5. AIA Consent to Treat form (parent signature)

6. AIA Position Statement (parent and student signature)

7. Proof of Health Insurance (student insurance is available if needed)

8. Booster Club Form

9. Athletic Trip Consent Form A (required for each sport - parent and student signature)

10. Copy of a birth certificate if you are a Freshmen or new student.

11. Brainbook Concussion Class Certificate (aiaonline.org) if you are a Freshmen or new athlete.

 

NEW FOR 21-22: All athletes must take the Opioid Class at aiaonline.org
